麝香保心丸穴位贴敷联合吞咽治疗仪治疗脑梗死后吞咽障碍的疗效观察(1)
【摘要】 目的 探讨麝香保心丸穴位贴敷联合吞咽治疗仪治疗脑梗死后吞咽障碍的疗效。 方法 84例脑梗死后吞咽障碍的患者， 采用随机数字表法分为对照组和观察组， 各42例。对照组采用吞咽神经肌肉电刺激仪和吞咽康复训练治疗， 观察组在对照组治疗基础上加用麝香保心丸穴位贴敷治疗， 比较两组治疗效果。结果 治疗前， 观察组患者洼田饮水试验评分为(4.21±0.54)分， 对照组患者洼田饮水试验评分为(3.96±0.68)分， 比较差异无统计学意义(P>0.05)。治疗14 d后， 观察组患者洼田饮水试验评分为(1.64±0.43)分， 对照组患者洼田饮水试验评分为(2.56±0.55)分;两组患者洼田饮水试验评分均低于治疗前， 且观察组低于对照组， 差异具有统计学意义(P<0.05)。观察组的总有效率92.86%高于对照组的73.81%， 差异具有统计学意义(P<0.05)。结论 麝香保心丸穴位贴敷联合吞咽治疗仪及吞咽康复训练治疗脑梗死后吞咽障碍， 能加快吞咽功能的恢复， 改善吞咽障碍， 值得临床推广应用。

【Abstract】 Objective To discuss the efficacy of Shexiang Baoxin pill acupoint application combined with swallowing therapy instrument on dysphagia after cerebral infarction. Methods A total of 84 dysphagia patients after cerebral infarction were divided by random number table method into control group and observation group， with 42 cases in each group. The control group was treated with swallowing neuromuscular electrical stimulation apparatus and swallowing rehabilitation training， and the observation group was treated with Shexiang Baoxin pill acupoint application on the basis of the control group. The treatment effect in two groups was compared. Results Before treatment， the observation group had water swallow test score as (4.21±0.54) points， which was， (3.96±0.68) points in the control group， and the difference was not statistically significant (P>0.05). Conclusion Shexiang Baoxin pill acupoint application combined with swallowing therapy instrument and swallowing rehabilitation training can accelerate the recovery of swallowing function and improve dysphagia after cerebral infarction， and it is worthy of clinical promotion and application.

吞咽障礙是脑梗死患者常见并发症之一， 大多数为假性球麻痹， 而假性球麻痹常由于多发性脑梗死或多次脑梗死引起， 在此类患者中约有30%～65%可出现吞咽障碍[1]。吞咽障碍可表现为吞咽困难、进食呛咳， 甚则不能自主吞咽， 容易噎住， 从而诱发了误吸、吸入性肺炎、营养不良、脱水、窒息等一系列合并症， 影响了脑梗死的康复， 甚至危及生命， 常规治疗效果一般。为探讨脑梗死后吞咽障碍的最佳疗法， 本科自2017年7月～2018年6月采用麝香保心丸穴位贴敷联合吞咽治疗仪对42例脑梗死后出现吞咽困难的患者进行治疗观察， 取得显著效果， 现报告如下。, 百拇医药(郑映娟 胡水勋 邓武锴 朱少君)
