【摘要】 目的 探讨甲状腺滤泡癌(FTC)和髓样癌(MTC)的超声声像图特征与术后病理的相关性。评价彩超在甲状腺癌鉴别诊断中的价值。方法
回顾性分析49例甲状腺癌病例,其中FTC21例,MTC28例,将其彩超表现和病理特征进行对照分析。结果 ①二维超声图像提示两者的密度、边界、钙化形式差异无统计学意义(P=0.202,P=0.318,P=0.286);②边界清晰与否决定于肿瘤浸润的类型,病理显示MTC多为无包膜广泛浸润型生长,边界不清晰;FTC多为包膜完整的微小浸润型生长,肿瘤边界一般清晰;③MTC(22/28)较FTC(3/21)更易发生颈部淋巴结转移(P<0.001)。结论 彩超对鉴别FTC和MTC有意义,彩超特征与病理改变有密切的相关性。
